Chickasaw Nation Residents

Welcome to the Chickasaw Nation Primary Care Clinic. We value you as a patient and look forward to serving you and your health needs.

Your health care team serves you by providing, overseeing and coordinating your overall wellness. Our objective is to replace episodic care based on symptoms and illnesses with coordinated, whole-person care and long-term, healing relationships. Our goal is to work together to facilitate a partnership to provide you with the best quality and comprehensive care by:

  • Clearly communicating your diagnosis, treatment options and results in a way that is easy to understand.
  • Actively listening to your concerns and questions to make informed decisions about your care.
  • Comprehensive guidance and clear instructions about available treatment options and ensuring you have a full understanding of your treatment goals and future plans.
  • Preventative care by offering screenings, immunizations and lifestyle guidance to help prevent illness and maintain your well-being.

As our patient you will have an ongoing relationship with our team and we will take collective responsibility for your care. Your responsibilities are to:

  • Ask questions, share your feelings and take an active part in your care
  • Be honest about your history, symptoms and other important information, including any changes in your health and well-being
  • Take all of your medications as directed and communicate with your health care team if you feel changes are needed
  • Inform us when there is a problem with the medication that you are taking
  • Make healthy decisions about your daily habits and lifestyle
  • Keep an open mind about suggestions to improve your health and well-being
  • Keep your scheduled appointments or reschedule in advance whenever possible

The health and wellness of our patients is our focus and providing the best possible care to every patient is our primary goal. We can meet this goal if we work together.

Primary care appointments must be made by calling the clinics. Appointments, prescription refills or test results cannot be made or given via email or other messaging methods.
