The Notice of Privacy Practices outlines patient rights regarding his/her protected health information and how the Chickasaw Nation Department of Health (CNDH) uses and discloses such information. The Authorization for Release of Protected Health Information is the document completed by the patient or the patient’s legal representative for obtaining a copy of the patient’s health information.
The document has specific elements that must be completed prior to the request being fulfilled. Upon completion of the Authorization, please submit to the Chickasaw Nation Medical Center (CNMC) Health Information Management Department, 1921 Stonecipher Blvd., Ada, Oklahoma 74820. Please allow 7–10 business days to receive your requested health information.

HIPAA is the federal Health Insurance Portability and Accountability Act passed in 1996. It requires health plans to protect the confidentiality of personal medical records and limit the personal health information released to others.
The Chickasaw Nation has evaluated policies and procedures across the health system and has taken steps to enhance privacy and security procedures already in place.

The compliance department is a required function for the Chickasaw Nation Department of Health (CNDH), ensuring adherence to tribal, federal and state regulations across our health care system. The CNDH Compliance Department promotes a culture of accountability and integrity. This helps meet legal requirements while aligning with the mission, vision and core values, and ensuring CNDH consistently provides safe and effective care to all patients across all facilities.
As a health care system receiving federal funding, CNDH is obligated to comply with specific standards and regulations to ensure safe and fair care for all patients. These include:
- Patient Rights and Responsibilities: Federal funding requires that we inform patients of their rights and responsibilities, emphasizing the importance of patients understanding both what they are entitled to and the expectations we hold them to. - Nondiscrimination and Accessibility Notice: Ensuring compliance with federal laws such as the Affordable Care Act (ACA), we are obligated to provide non-discriminatory services and ensure accessibility for all patients, regardless of their background or abilities. ***Note: see attached CTR link: (MAN 07 COM 1001) PATIENT BILL OF RIGHTS
- EMTALA Notice: As a recipient of federal funding, we are obligated under the Emergency Medical Treatment and Labor Act (EMTALA) to provide emergency care to anyone seeking treatment in our Emergency Department, including those who come to the CNMC and are in labor, regardless of their ability to pay. This ensures that no patient is denied care in an emergency or during labor. ***Note: see attached EMTALA Notice/word document (there is no CTR link for the EMTALA signage).
