This program assists families who are caring for an older relative with a chronic illness or disability, or grandparents raising young children. It provides five areas of service for the caregiver, including respite care.

Participants must be caring for those who are chronically ill or have a life-altering physical, mental or developmental disability, or older individuals caring for children under 18 years of age. Additional eligibility requirements may apply.

  • Those who have a life-altering physical, mental or developmental disability
  • Grandparents raising a grandchild under 18 years of age
  • Those who rely on a caregiver for at least two activities of daily living needs

Program Details

This is a federal program funded through the Administration on Aging, Title VI-C program to assist families who are caring for an older relative with a chronic illness or disability and to help provide support services for family caregivers and/or grandparents or older individuals who are relative caregivers.

This program provides five areas of service for the caregiver, including respite care. Referral forms are available at any senior center and can be completed via telephone. Applicants can expect a home visit to determine eligibility. If applicant qualifies, he/she will receive notification within 10 business days.
