Application Available

This program assists families who are caring for an older relative with a chronic illness or disability, or grandparents raising young children. It provides five areas of service for the caregiver.

Participants must be caring for those who are chronically ill or have a life-altering physical, mental or developmental disability, or older individuals caring for children under 18 years of age. Additional eligibility requirements may apply.

Eligibility
  • Caring for a frail elder age 60 or older
  • Caring for those who are chronically ill
  • Those who have a life-altering physical, mental or developmental disability (Age 18-58)
  • Grandparents raising a grandchild under 18 years of age; Kinship raising kin under 18 years of age
  • Those who rely on a caregiver for at least two activities of daily living needs
  • Active citizen of a federally recognized tribe

Program Details

This is a federal program funded through the Administration on Aging, Title VI, Part C program to assist families who are caring for an older relative with a chronic illness or disability and to help provide support services for family caregivers and/or grandparents or older individuals who are relative caregivers.

This program provides five areas of service for the caregiver. Applicants can expect a home visit to determine eligibility. If applicant qualifies, he/she will receive notification within 10 business days.
