What happens at Empowered Living clinic appointments?
Ask your personal care provider if your child qualifies for Empowered Living.
Participants can obtain a referral to the pediatric Empowered Living clinic from a Chickasaw Nation health care provider. Parents and guardians may also call the Empowered Living program directly to set up an appointment.
The Empowered Living clinic is a multidisciplinary, referral-based clinic that provides children, adolescents and teens specialized services and resources to assist them in achieving healthy lifestyle goals.
