Life Rx Program


Out of an abundance of caution and to mitigate the spread of the coronavirus, or COVID-19, certain Chickasaw Nation events, classes, facilities, etc., will be postponed, canceled or closed. A yearlong lifestyle change initiative proven to help prevent and delay the onset of type 2 diabetes, Life Rx encourages participants to make lasting behavior changes involving nutrition, physical activity and coping strategies. Classes are instructed by a trained lifestyle coach.

This program is for pre-diabetics and those with specific risk factors. It does not treat or provide information for those already diagnosed with type 1 or type 2 diabetes. If you have type 1 or type 2 diabetes, talk to your primary care provider to learn about other services such as the diabetes care center, that may be right for you.


  • 18 years of age or older
  • BMI (body mass index) of 24 or higher
  • Must have one or more of the following:
    • Positive screening for prediabetes based on the CDC Prediabetes Screening Test
    • History of gestational diabetes mellitus (diabetes while pregnant)
    • Prediabetes diagnosis confirmed by a blood glucose test within the past 12 months
