LifeRx equips participants to make lasting behavior changes involving nutrition, physical activity and coping strategies. This program is for those with pre-diabetes and specific risk factors.

Participants must be 18 years of age or older. Additional eligibility requirements may apply.

Eligibility

This program is for those who are 18 years of age or older and have at least one of the following:

  • Prediabetes diagnosis confirmed by a blood test within the past 12 months
  • History of gestational diabetes mellitus (diabetes while pregnant)
  • Elevated risk level (determined after application is reviewed)
    • This program is not suitable for diabetes treatment or diabetes self-management. If you currently have type 1 or type 2 diabetes, you will not be eligible for this program. Talk to your primary care provider to learn about other options that may be right for you.

Program Details

A yearlong lifestyle change initiative proven to help prevent and delay the onset of type 2 diabetes, LifeRx encourages participants to make lasting behavior changes involving nutrition, physical activity and coping strategies. Classes are instructed by a trained lifestyle coach.

This program is for pre-diabetics and those with specific risk factors. It does not treat or provide information for those already diagnosed with Type 1 or Type 2 diabetes. If you have Type 1 or Type 2 diabetes, talk to your primary care provider to learn about other services such as the diabetes care center, that may be right for you.
