The community health representative program provides First Americans with medically guided primary healthcare services when no other resource is available. Participants must have tribal citizenship, be homebound or disabled and reside within the Chickasaw Nation.

Program Details

This community health representative program promotes the highest possible health level of Native Americans by providing medically-guided primary health care services where no other resource is available. Services offered by the CHR program include: patient advocacy, liaison between clients and tribal and community resources, patient education and health promotion/disease prevention.

The Chickasaw Nation Division of Health offers a variety of programs and services at several convenient locations throughout our 13-county jurisdiction. The Chickasaw Nation Medical Center (CNMC) is located in Ada, Oklahoma, and outlying clinics are located in Ardmore, Durant, Purcell and Tishomingo.
