Must be a Chickasaw citizen and have a documented physical or mental disability.

Completed special needs assistance application form
Copy of applicant’s Chickasaw citizenship
Copy of applicant’s current Individualized Education Plan (IEP) or documentation of disability from a physician
Receipts for disability related assistive technology needed or expenses paid to allow the applicant to participate in an extracurricular activity
