The MAP program provides tribal citizens assistance with up to two episodes each year for unmet medical care not to exceed $2,500 per incident. The MAP program is a supplement to contract health services designed to provide necessary medical services to support on-going care from a CNDH provider to maximize the medical treatment plan.

Required applications are available at the address above, can be sent via email or may be obtained at all CNDH clinics.

Application must be completed within 180 days from the incident/injury/illness.

You Provide:

  • Chickasaw citizenship card
  • Copy of insurance
  • Most recent original statements, script and/or referral
  • If referred, must provide a copy of denial letter from contract health

Eligibility:

  • Service must not be provided by CNDH facilities
  • Must have a medical necessity

To be eligible to participate in the MAP program, citizens must have an active chart within CNDH. (An active chart is defined as having received services within the past three years from the date of application.) Citizens must also have a referral denial from Medisaw. The MAP program is utilized as a last resort, therefore, alternate resources must be exhausted. The program does not cover services that are provided within the CNDH.
