The eye clinic provides examination, diagnosis and management of conditions of the eye.  Management of diabetic retinopathy and cataract surgery is available for citizens eligible for the Medical Assistance Program (MAP).  Required forms are available at the above address and all CNDH clinics.

• If residing outside of service area, must provide a copy of denial letter from contract health

Eye appointments are made the first Wednesday of each month between 8 a.m. and 8:30 a.m. either in person or by phone.  The eye appointment will be scheduled for the next month.  When making eye appointments, all patients will be required to give their chart number or Social Security number.
