About:
This program provides medical transportation as well as pick-up and delivery of prescriptions.

The transportation services department has drivers located in Ada, Ardmore, Duncan, Purcell, Sulphur and Tishomingo. In most cases, transportation can be provided to medical appointments the same day the request for service is made.

You Provide:

CDIB or citizenship card

Eligibility:

  • Must reside within the Chickasaw Nation
  • Must possess a Certificate Degree of Indian Blood (CDIB) from a federally recognized tribe and/or a Chickasaw Nation citizenship card
  • Must be a non-emergency medical transport. If not medical transport, the request must be approved by the transportation director
  • Must be at least 18 years of age unless accompanied by a parent or legal guardian
  • All transports will be scheduled on a first call, first serve basis
  • Must receive prior approval for transport outside of the Chickasaw Nation boundaries
  • Must schedule at least 24 hours in advance or 72 hour notice required for and appointment out of the area
  • Must call at least 24 hours in advance for prescription pick up or delivery

  • 24-hour advance notice
  • 72-hour advance notice for Oklahoma City appointments

Appointments should be made between 8:30 a.m. and 2:00 p.m. Monday - Friday.

The mission of transportation services is to provide transportation and prescription pickup and delivery to all Native Americans within the Chickasaw Nation, in a comfortable, simple and easy to access fashion.
