About:
This service provides assistance with basic housekeeping duties. Referral forms are available above or any senior nutrition center and can be completed via telephone.

An applicant can expect an elderly specialist to make an initial home visit to assess the services needed. Once approved, the chore manager will contact the client.

You Provide:

• Citizenship card (client)

Eligibility:

• Must be Chickasaw
• Must be 60 years of age or older
• Must reside within the Chickasaw Nation

Restrictions:

• Client must be at home at time of service
• Client will be assessed on a biannual basis
• Verification that services are not being duplicated
  (home health, hospice, other)
• Services requested must be chores the clients themselves are
  unable to do
• Priority is given to clients who live alone
