Pharmacy Services and Anticoagulation and Hyperlipidemia Clinics

Outpatient Hours of Operation:

Clinics: Monday – Friday, 8 a.m. – 5 p.m.

CAIHF Outpatient Pharmacy Refill Center:

Monday – Friday, 8 a.m. – 8 p.m.
Saturday and Sunday, 10:30 a.m. – 7:00 p.m.

Inpatient Hours of Operation:

Available on-call 24 hours a day, 7 days a week

The pharmacy is available to safely dispense the medications prescribed by your CNDH provider. Pharmacists work hand-in-hand with doctors to ensure patients receive the best possible drug treatment and the pharmacist is available to compliment the instructions provided by your doctor and/or nurse. The required form is available from medical records or at the address above.

To be eligible for pharmacy services, patients must have a CDIB or citizenship card from a federally recognized tribe. Prescriptions must be written by a CNDH provider.

The anticoagulation and hyperlipidemia clinics provide monitoring and education to patients taking anticoagulation and cholesterol lowering medications. Patients receiving this service must have an active chart and a referral by a CNDH provider.
