Medication not carried in the tribal pharmacy may be provided by this program after being prescribed by a CNDH provider. No age restriction is applied if patient has an active medical chart
within the health system. There are several additional restrictions for receiving this service. Required forms are available at the above address.

• Drug not on CNDH formulary
