available at the above address.

You Provide:

• Driver’s license
• Social Security card
• Referral from agency (if applicable)
• Case history/narrative, police reports, protective orders, medical reports
• Statements for requested assistance
• Income verification (if applicable)
• CDIB (if Native American)

Eligibility:

• Must be current or recent victim of family violence
• Residing in or relocating to the Chickasaw Nation
