About

This program includes benefits for youth, disabled citizens, elders and citizens of all ages with private insurance drug coverage to allow prescription submission from the citizen’s provider.

Covered prescriptions may be supplied by mail order pharmacy. Participation does not require an active chart within CNDH. During enrollment the prescription medication required will be identified and pre-authorized by the coordinator. Once enrolled, citizens will send the original prescription to the refill center where it will be processed and mailed to them. As refills are needed, citizens will then call the pharmacy refill center directly to request medications each month.

Private insurance drug coverage or Medicare Part D is required for adults eligible for the program. Insurance is not required for youth less than 19 years of age. The Health Spending Account may be used to pay for monthly premiums. The pharmacy will file third party insurance claims and the patient will have no out-of-pocket expenses.

Citizen may pre-enroll 90 days before Medicare Part D is effective.

Featured Program Expansions:
Refrigerated medications can be mailed to citizens as a part of the Prescription Mail Order Program.

Non-covered items:
Anti-rejection drugs; chemotherapy; controlled substances; investigational drugs; IV medications/ devices to administer; over the counter medications; performance enhancing drugs and short-term use drugs.

You Provide
  • CDIB and Chickasaw citizenship card
  • Original prescription provider information
  • Copy of private insurance card

Eligibility

  • 65 years of age or older; or
  • Disabled and on Medicare Part D; or
  • Youth up to 18 years of age, without an active chart within CNDH
    (Active chart is defined as having received services within the
     past 3 years from date of application); or
  • Citizens of all ages with private insurance drug coverage
