This is a residential and educational care facility for Native American children.

• Current contact list
• Copy of insurance, Medicaid or Sooner Care

Required forms are available above or can be mailed upon request. Applications are taken year-round until enrollment is full. An applicant should know within three days if he/she qualifies for the program.
